    Athletic and School Physical Dates at HCMH Clinics:
    Special dates for athletic and school physicals will be offered in June and July at the Britt and Garner Medical Clinics. Exams will be $30.00 each during the special dates and insurance will not be filed. Physical forms may be downloaded below.

    We encourage you to make your appointments early to be assured of an appointment.

    Parents: Please mark one of the options on the adolescent health supervision form and sign. This signed form along with the athletic physical form will need to be presented at the time of service.

    If other healthcare concerns are addressed during this visit, please be aware that our facility will be billing your insurance for this or if you are self-pay, will be sending you a bill.
